FCC COVID-19 Telehealth Program Application Process

On Monday April 13, the FCC application portal for the COVID-19 Telehealth Program was launched with remote monitoring included. Carevive is excited to help eligible health care providers apply for the first-come serve COVID-19 Telehealth Program.

The Program will provide immediate support to eligible health care providers responding to the COVID-19 pandemic by fully funding their telecommunications services, information services, and devices necessary to provide critical connected care services until the program’s funds have been expended or the COVID-19 pandemic has ended.

Program Application Process Key Steps

File for a Request for Funding

Obtain an eligibility determination  for each site on the application from the Universal Service Administrative Company (USAC) by filing FCC Form 460 through “My Portal” on USAC’s webpage.  Applicants that do not yet have the eligibility determination from USAC can still file an application with the Commission while their FCC Form 460 is pending with USAC.

Register for an FFC Registration Number (FRN) in the Commission Registration System (CORES).

Submit an application for funding through the COVID-19 Telehealth Program to the Commission the the Commission’s online portal

Receive Payment

Register with the System of Award Managment (SAM). Applicants are strongly encouraged to begin this process before they receive a funding decision, however it is not required for submitting the application or funding decision.
