Sidney Kimmel Cancer Center to kickoff Survivorship Care Planning

Sidney Kimmel Cancer Center at JeffersonThe Thomas Jefferson University Sidney Kimmel Cancer Center will use a novel platform to deliver survivorship care plans. By using Carevive’s Care Planning System (CPS), Jefferson will be able to provide personalized survivorship care plans to its patients across multiple tumor types. The Carevive CPS will integrate with Jefferson’s electronic health record (EHR) and registry software.

Registry data can be used to automate the Treatment Summary. Patient and PCP versions of care plans can be generated and Carevive’s reporting functionality are used to improve care coordination across disciplines.

Supported by a scientifically rigorous and evidence-based rules-based cloud server, Carevive is able to deliver accurate and personalized care plans to patients at the point of care. Carevive integrates with cancer registries and hospital EHR systems by leveraging HL7 interfaces.

CCO and co-founder Dr. Carrie Stricker states that clinical content is authored and maintained by thousands of oncology experts across the U.S. “Our software eliminates the need for clinical staff to manually develop and maintain information for patients on treatment options, symptom prophylaxis and management, and survivorship education across numerous cancer types, and also avoids the need for IT teams to configure and update these templates in EHRs, ” she said. “All of our care planning tools can be localized for cancer center service line offerings.
